Narrative:

After the route brief was accomplished at smf; we received a runway change from 34L to 16R. Captain reprogrammed the FMC while I [worked up the takeoff numbers]. After completion; I re-verified the waypoints from the new runway to where it met the common routing from either runway. After departure after frogo; ATC asked if we were cleared direct rebrg. We initially said we were flying our filed route but then discovered fra missing. It should have been frogo; fra; and rebrg. Fra was there when we did the original route brief; but somehow was dropped when we changed to runway 16R. ATC said the same thing happened to a previous company aircraft ahead of us.
