Narrative:

I was attempting a post annual inspection test flight with a mechanic as a passenger. At about 150-200' AGL we experienced engine failures as the gear was raising, resulting in a forced landing in the overrun on departure end of 29R. I have not reviewed sign off of the aircraft and engine logs and cause of failures is still unknown. It was determined I had inadvertently flown with expired medical and biennial flight review. Medical was renewed 8/89 and am making arrangements for my biennial review.

Title: DOUBLE ENGINE FAILURE IMMEDIATELY AFTER TKOF. FORCED LNDG.

Narrative: I WAS ATTEMPTING A POST ANNUAL INSPECTION TEST FLT WITH A MECH AS A PAX.
