Narrative:

Captain shooting ILS to 22 at lga. We were given clearance to cross 15 out at 4000, 10 out at 3000, cleared for the approach. I read back clearance, stumbled over the numbers, and reread it back correctly. We already had 4000 set in the altitude alert and as we approached 4000' actual at 16 1/2 DME, I realized the captain was not going to level off even tho he had called 5000 for 4000 during the descent. I called out '4000' is the restriction till 15!' we only had one DME readout on my side and when he saw 15 1/2 he thought it was 14 1/2 and continued descent. At that point the second officer and jump set rider both were calling out 4000' but he pressed on. We crossed 15 DME at 3450' MSL. This was an involved CRM (cockpit resource management) problem.

WE ALREADY HAD 4000 SET IN THE ALT ALERT AND AS WE APCHED 4000' ACTUAL AT 16 1/2 DME, I REALIZED THE CAPT WAS NOT GOING TO LEVEL OFF EVEN THO HE HAD CALLED 5000 FOR 4000 DURING THE DSCNT. I CALLED OUT '4000' IS THE RESTRICTION TILL 15!' WE ONLY HAD ONE DME READOUT ON MY SIDE AND WHEN HE SAW 15 1/2 HE THOUGHT IT WAS 14 1/2 AND CONTINUED DSCNT. AT THAT POINT THE S/O AND JUMP SET RIDER BOTH WERE CALLING OUT 4000' BUT HE PRESSED ON. WE CROSSED 15 DME AT 3450' MSL. THIS WAS AN INVOLVED CRM (COCKPIT RESOURCE MGMNT) PROBLEM.
