Narrative:

I was working the arrival east sector at clt TRACON. I was working small aircraft X at 4000' inbound to a satellite airport in clt airspace. I pointed small aircraft X out to departure sector since small aircraft X would transit through 14 NM of departure's airspace. Departure was working air carrier Y eastbound. I noticed the mode C readout of 3400-3500 and queried the departure controller. Departure controller informed me that he forgot about small aircraft X. We both took evasive action. Small aircraft X saw the air carrier Y. I then instructed small aircraft X to maintain visual separation and cleared the aircraft on course. The aircraft missed each other by approximately 1 NM. The departure controller was busy at the time. He should have had an assistant helping him. But we don't have enough controllers. I first considered handing off small aircraft X to departure but elected to keep the aircraft to ease his workload.

DEP CTLR INFORMED ME THAT HE FORGOT ABOUT SMA X. WE BOTH TOOK EVASIVE ACTION. SMA X SAW THE ACR Y. I THEN INSTRUCTED SMA X TO MAINTAIN VISUAL SEPARATION AND CLRED THE ACFT ON COURSE.
