Narrative:

Piarco center failed to coordinate a flight at 35,000'. The aircraft entered san juan airspace without knowledge to the san juan controller. The problem was discovered when an commercial radio message was received with a position report inside sjv airspace. At that time it was discovered that the aircraft was in direct conflict with another aircraft. Action was taken to climb the first aircraft but separation was lost already.

Title: ACR ENTERED ADJACENT AIRSPACE WITHOUT COORD OR HANDOFF RESULTING IN LESS THAN STANDARD SEPARATION WITH ANOTHER ACFT AT THE SAME ALT.

Narrative: PIARCO CENTER FAILED TO COORDINATE A FLT AT 35,000'. THE ACFT ENTERED SAN JUAN AIRSPACE WITHOUT KNOWLEDGE TO THE SAN JUAN CTLR. THE PROBLEM WAS DISCOVERED WHEN AN COMMERCIAL RADIO MESSAGE WAS RECEIVED WITH A POSITION REPORT INSIDE SJV AIRSPACE. AT THAT TIME IT WAS DISCOVERED THAT THE ACFT WAS IN DIRECT CONFLICT WITH ANOTHER ACFT. ACTION WAS TAKEN TO CLIMB THE FIRST ACFT BUT SEPARATION WAS LOST ALREADY.
