Narrative:

Air carrier X departed luf AFB on lake departure. Aircraft was assigned runway heading by luke AFB controller and supposedly had an intermediate altitude assignment. The center controller expected aircraft to turn onto lake departure and climb to 10,000'. The center controller restated to aircraft to maintain 10,000' and asked air carrier X if he had been cleared by luke RAPCON. Air carrier X advised he had been told to contact center by luke. Air carrier X got within 3 NM of inbound aircraft to phx approximately 8 NM north of gbn, az. Upon further investigation, luke RAPCON did not talk to air carrier X, had not assigned anything other than runway heading and to maintain 5000'. RAPCON did give radar handoff to center but luke tower put air carrier X on center frequency. LOA states that aircraft shall be on established pdr and assigned 10,000' MSL. Center controller assumed this to be the case.

Title: DUE TO COORD PROBLEMS BETWEEN FACS, A DEPARTING ACFT CAME WITHIN 3 MILES OF ANOTHER ACFT.

Narrative: ACR X DEPARTED LUF AFB ON LAKE DEP. ACFT WAS ASSIGNED RWY HDG BY LUKE AFB CTLR AND SUPPOSEDLY HAD AN INTERMEDIATE ALT ASSIGNMENT. THE CENTER CTLR EXPECTED ACFT TO TURN ONTO LAKE DEP AND CLIMB TO 10,000'. THE CENTER CTLR RESTATED TO ACFT TO MAINTAIN 10,000' AND ASKED ACR X IF HE HAD BEEN CLRED BY LUKE RAPCON. ACR X ADVISED HE HAD BEEN TOLD TO CONTACT CENTER BY LUKE. ACR X GOT WITHIN 3 NM OF INBND ACFT TO PHX APPROX 8 NM N OF GBN, AZ. UPON FURTHER INVESTIGATION, LUKE RAPCON DID NOT TALK TO ACR X, HAD NOT ASSIGNED ANYTHING OTHER THAN RWY HDG AND TO MAINTAIN 5000'. RAPCON DID GIVE RADAR HANDOFF TO CENTER BUT LUKE TWR PUT ACR X ON CENTER FREQ. LOA STATES THAT ACFT SHALL BE ON ESTABLISHED PDR AND ASSIGNED 10,000' MSL. CENTER CTLR ASSUMED THIS TO BE THE CASE.
