Narrative:

I issued a clearance through pih tower 'mlt X cleared to slc as filed climb and maintain 15,000'. I issued a code. One minute later I entered a departure message in the computer. The computer sent me a message to change the assigned code of mlt X. Three minutes later the aircraft departed. He checked on frequency out of 6800 climbing to 15,000. I answered 'roger'. I failed to start a track on the aircraft. In addition, the track did not automatic-start because the aircraft was on the wrong code. The aircraft entered the next controller's airspace without coordination or handoff. He finally entered salt lake approach airspace without a handoff. I realized the mistake about the same time the aircraft entered the approach airspace. I tried to make coordination but salt lake had already identified him.

Title: MLT FLEW THROUGH ADJACENT SECTOR AIRSPACE AND PENETRATED APCH AIRSPACE WITHOUT A HANDOFF OR POINTOUT.

Narrative: I ISSUED A CLRNC THROUGH PIH TWR 'MLT X CLRED TO SLC AS FILED CLIMB AND MAINTAIN 15,000'. I ISSUED A CODE. ONE MINUTE LATER I ENTERED A DEP MESSAGE IN THE COMPUTER. THE COMPUTER SENT ME A MESSAGE TO CHANGE THE ASSIGNED CODE OF MLT X. THREE MINUTES LATER THE ACFT DEPARTED. HE CHECKED ON FREQ OUT OF 6800 CLIMBING TO 15,000. I ANSWERED 'ROGER'. I FAILED TO START A TRACK ON THE ACFT. IN ADDITION, THE TRACK DID NOT AUTO-START BECAUSE THE ACFT WAS ON THE WRONG CODE. THE ACFT ENTERED THE NEXT CTLR'S AIRSPACE WITHOUT COORD OR HANDOFF. HE FINALLY ENTERED SALT LAKE APCH AIRSPACE WITHOUT A HANDOFF. I REALIZED THE MISTAKE ABOUT THE SAME TIME THE ACFT ENTERED THE APCH AIRSPACE. I TRIED TO MAKE COORD BUT SALT LAKE HAD ALREADY IDENTIFIED HIM.
