Narrative:

I contacted springfield approach control on 121.1 MHZ and informed them of my position, type aircraft, altitude and intentions to land at 3DW. I again contacted springfield approach to notify them of 3DW in sight 5 mi. They acknowledged and told me to squawk 1200 and frequency change approved. I then asked them if they could close my flight plan. They said no and instructed me to contact springfield FSS on 122.2 MHZ or call columbia FSS by telephone after landing. I said thanks and switched to 122.2 and contacted springfield FSS and asked them to close my flight plan. The female controller responded affirmatively and conversation ended. I then switched to 122.8 MHZ and announced my position and intentions to 3DW traffic. The following morning, I found a letter from the cap in the pilot's seat of my aircraft parked on the ramp at dfw where I tied it down the previous night. It seems that columbia FSS never got the message that my flight plan had been closed. I immediately called springfield FSS and spoke with the same controller I had closed my flight plan with the evening before. I was told briefly that it was an error in procedure and communication between themselves and columbia FSS and apologized and stated that controller had contacted columbia FSS and told them that I had done everything I was supposed to and had done it properly. The matter should be closed as far as I am concerned. However, a lot of people, according to the cap were needlessly disturbed by this incident. I regret that this happened and feel bad about the entire matter. Upon reflection of the entire series of events from the filing of my flight plan to tie down, I know that I don't have any reason to feel that way about it as I did contact flight service and closed my flight plan as required and in accordance with proper procedures. However, there is one thing I could have done that would have saved a lot of people a lot of unnecessary trouble. I could have provided FSS with a destination phone number. This item is optional on the flight plan forms and controllers don't ask for it when you file. I have to call my flight plans in as the nearest FSS is 30 mi from my home base at siloam springs, ar and I almost always follow the form from memory as I can never find one at slg. From now on, I provide a destination phone number if at all available because mistakes happen and the more information FSS has to work with, the easier it is to correct a problem as well as realize that there is a problem that needs further attention.

Title: CALLED AND CLOSED VFR FLT PLAN WITH FSS. NEXT DAY NOTIFIED IT HAD NOT BEEN CLOSED AND MANY FOLKS HAD BEEN NEEDLESSLY DISTURBED BY INCIDENT. FSS APOLOGIZED AND INDICATED IT WAS INTERNAL COORD PROBLEM.

I AGAIN CONTACTED SPRINGFIELD APCH TO NOTIFY THEM OF 3DW IN SIGHT 5 MI. THEY ACKNOWLEDGED AND TOLD ME TO SQUAWK 1200 AND FREQ CHANGE APPROVED. I THEN ASKED THEM IF THEY COULD CLOSE MY FLT PLAN. THEY SAID NO AND INSTRUCTED ME TO CONTACT SPRINGFIELD FSS ON 122.2 MHZ OR CALL COLUMBIA FSS BY TELEPHONE AFTER LNDG. I SAID THANKS AND SWITCHED TO 122.2 AND CONTACTED SPRINGFIELD FSS AND ASKED THEM TO CLOSE MY FLT PLAN. THE FEMALE CTLR RESPONDED AFFIRMATIVELY AND CONVERSATION ENDED. I THEN SWITCHED TO 122.8 MHZ AND ANNOUNCED MY POSITION AND INTENTIONS TO 3DW TFC. THE FOLLOWING MORNING, I FOUND A LETTER FROM THE CAP IN THE PLT'S SEAT OF MY ACFT PARKED ON THE RAMP AT DFW WHERE I TIED IT DOWN THE PREVIOUS NIGHT. IT SEEMS THAT COLUMBIA FSS NEVER GOT THE MESSAGE THAT MY FLT PLAN HAD BEEN CLOSED. I IMMEDIATELY CALLED SPRINGFIELD FSS AND SPOKE WITH THE SAME CTLR I HAD CLOSED MY FLT PLAN WITH THE EVENING BEFORE. I WAS TOLD BRIEFLY THAT IT WAS AN ERROR IN PROC AND COM BETWEEN THEMSELVES AND COLUMBIA FSS AND APOLOGIZED AND STATED THAT CTLR HAD CONTACTED COLUMBIA FSS AND TOLD THEM THAT I HAD DONE EVERYTHING I WAS SUPPOSED TO AND HAD DONE IT PROPERLY. THE MATTER SHOULD BE CLOSED AS FAR AS I AM CONCERNED. HOWEVER, A LOT OF PEOPLE, ACCORDING TO THE CAP WERE NEEDLESSLY DISTURBED BY THIS INCIDENT. I REGRET THAT THIS HAPPENED AND FEEL BAD ABOUT THE ENTIRE MATTER. UPON REFLECTION OF THE ENTIRE SERIES OF EVENTS FROM THE FILING OF MY FLT PLAN TO TIE DOWN, I KNOW THAT I DON'T HAVE ANY REASON TO FEEL THAT WAY ABOUT IT AS I DID CONTACT FLT SERVICE AND CLOSED MY FLT PLAN AS REQUIRED AND IN ACCORDANCE WITH PROPER PROCS. HOWEVER, THERE IS ONE THING I COULD HAVE DONE THAT WOULD HAVE SAVED A LOT OF PEOPLE A LOT OF UNNECESSARY TROUBLE. I COULD HAVE PROVIDED FSS WITH A DEST PHONE NUMBER. THIS ITEM IS OPTIONAL ON THE FLT PLAN FORMS AND CTLRS DON'T ASK FOR IT WHEN YOU FILE. I HAVE TO CALL MY FLT PLANS IN AS THE NEAREST FSS IS 30 MI FROM MY HOME BASE AT SILOAM SPRINGS, AR AND I ALMOST ALWAYS FOLLOW THE FORM FROM MEMORY AS I CAN NEVER FIND ONE AT SLG. FROM NOW ON, I PROVIDE A DEST PHONE NUMBER IF AT ALL AVAILABLE BECAUSE MISTAKES HAPPEN AND THE MORE INFO FSS HAS TO WORK WITH, THE EASIER IT IS TO CORRECT A PROBLEM AS WELL AS REALIZE THAT THERE IS A PROBLEM THAT NEEDS FURTHER ATTN.
