Narrative:

1) I presented myself to the flight examiner, on 4/wed/89, for a private pilot flight test. 2) my log book indicated a x-country flight on 3/tue/89, originating at erie; 48V to publication, and a return flight from publication to 48V. 3) he indicated that I had possibly violated far's, because he could not find an endorsement in my log book for the flight from 48V to publication and return flight from publication to 48V on 3/tue/89. 4) he indicated to me that he was going to submit a report of a possible violation of the far's to the FAA flight standards division. 5) the missing endorsement had been placed in my log book on the back inside cover and was written on a piece of scotch magic tape which I used to secure a spare door and magneto key to the back inside cover of the log book. My flight instrument wrote the body of endorsement for the x-country flight from 48V to publication to 48V on 3/tue/89 on this strip of tape, with his signature written on the actual log book material. When I removed the spare key from my log book on 3/thur/89, to give it to tri county instrument shop at 48V, so that they could do work on my transponder altitude encoder, I removed the key and the strip of tape with the x-country endorsement on it. I then inadvertently discarded the strip of tape with the endorsement on it. 6) corrective steps to be taken to ensure that a situation of this nature will not occur again are: a) replace my old tattered, torn and pages dislodged and loose from the spine log book with a new log book. (I got the old log book when I was introduced to aviation in 1959!) B) not tape or store keys, flat objects and other non-incidental objects to or in my new log book. C) adhere to a professional attitude when logging flight time. D) be sure that all endorsements are attached and current before any flight operation.

Title: REPORTER ADVISED OF A POSSIBLE FEDERAL AVIATION REG VIOLATION BECAUSE OF ENDORSEMENT MISSING IN STUDENT PLT LOG BOOK.

Narrative: 1) I PRESENTED MYSELF TO THE FLT EXAMINER, ON 4/WED/89, FOR A PVT PLT FLT TEST. 2) MY LOG BOOK INDICATED A X-COUNTRY FLT ON 3/TUE/89, ORIGINATING AT ERIE; 48V TO PUB, AND A RETURN FLT FROM PUB TO 48V. 3) HE INDICATED THAT I HAD POSSIBLY VIOLATED FAR'S, BECAUSE HE COULD NOT FIND AN ENDORSEMENT IN MY LOG BOOK FOR THE FLT FROM 48V TO PUB AND RETURN FLT FROM PUB TO 48V ON 3/TUE/89. 4) HE INDICATED TO ME THAT HE WAS GOING TO SUBMIT A RPT OF A POSSIBLE VIOLATION OF THE FAR'S TO THE FAA FLT STANDARDS DIVISION. 5) THE MISSING ENDORSEMENT HAD BEEN PLACED IN MY LOG BOOK ON THE BACK INSIDE COVER AND WAS WRITTEN ON A PIECE OF SCOTCH MAGIC TAPE WHICH I USED TO SECURE A SPARE DOOR AND MAGNETO KEY TO THE BACK INSIDE COVER OF THE LOG BOOK. I THEN INADVERTENTLY DISCARDED THE STRIP OF TAPE WITH THE ENDORSEMENT ON IT. 6) CORRECTIVE STEPS TO BE TAKEN TO ENSURE THAT A SITUATION OF THIS NATURE WILL NOT OCCUR AGAIN ARE: A) REPLACE MY OLD TATTERED, TORN AND PAGES DISLODGED AND LOOSE FROM THE SPINE LOG BOOK WITH A NEW LOG BOOK. (I GOT THE OLD LOG BOOK WHEN I WAS INTRODUCED TO AVIATION IN 1959!) B) NOT TAPE OR STORE KEYS, FLAT OBJECTS AND OTHER NON-INCIDENTAL OBJECTS TO OR IN MY NEW LOG BOOK. C) ADHERE TO A PROFESSIONAL ATTITUDE WHEN LOGGING FLT TIME. D) BE SURE THAT ALL ENDORSEMENTS ARE ATTACHED AND CURRENT BEFORE ANY FLT OPERATION.
