Narrative:

Departing dfw at xx apr 1989, we were climbing through 11,000' for 12,000' when departure control asked what our altitude was, we replied 11,000 for 12,000. He asked if he had given us clearance to 12,000, I replied no, we were cleared to 12,000' by clearance delivery, also when I checked in with departure I called 2000' for 12,000'. Because the published departure calls for 10,000 altitude, I questioned the copilot about it on taxi out. He said he was given 12,000' by clearance and read it back, which they acknowledged. No evasive action was required.
