Flight dispatched with 2 flight plans. Both flight plans common to gt. Filed flight plan was gt, red 763 bacus, AR8 EC6, etc. Standby (desired flight plan) was gt, A554 champ. ATC clearance received as follows: 'cleared to jfk climb and maintain FL350 on course.' due to the lack of clarity in our departure clearance and the NOTAM requesting a full route readback I replied, 'cleared to jfk, climb to and maintain FL350 on course, which is west 4 ribes, west 2 puerto plata, A554 sekar, A554 grand turk, R763 bacus...' santo domingo replied, 'readback correct.' while taxiing out santo domingo said, 'I have a change to your clearance, are you ready to copy?' I replied, 'affirmative.' santo domingo said, 'you are now reclred A554 to ny.' I read back, 'now cleared A554 to ny.' after takeoff santo domingo asked us to 'call established A554.' I replied with, 'roger, we are climbing through 2000' estimating ribes at 30.' no response received from santo domingo. Passing over ribes at 16000' we passed over an unk airliner (approximately 2000'). Unk airliner pilot was speaking spanish to santo domingo. Santo domingo then asked for our 'position and altitude.' we replied, 'ribes at FL160.' santo domingo replied, 'you were cleared A554 to call established.' we replied, 'roger we were, but we never called established and we gave you an estimate of ribes at 30.' the matter ended with our apology for whatever misunderstanding occurred.

WE REPLIED, 'ROGER WE WERE, BUT WE NEVER CALLED ESTABLISHED AND WE GAVE YOU AN ESTIMATE OF RIBES AT 30.' THE MATTER ENDED WITH OUR APOLOGY FOR WHATEVER MISUNDERSTANDING OCCURRED.
