Narrative:

Was receiving flight following from lga TRACON northwest sector at the time of near miss. Small transport was northeast bound level at 11500' direct from 3nj6 inductotherm airport, rancocas, nj, to bdl in good VFR conditions, squawking XXXX altimeter setting 30.25. At approximately 10 NM northwest of lga, air carrier flight widebody transport was cleared to 12000' on 270 degree heading, climbed from under the right front nose of my aircraft through my altitude when I first saw him. I initiated an immediate descent to 11000' to stay clear of him. We had been on a near collision course. Callback conversation with reporter revealed the following: learned reporter called TRACON after landing and they apologized for the error. Controller was removed from the position. Reporter later saw ntap and transcript. Was so close specialist could not tell which side of small transport widebody transport passed and both altitude readouts went into coast. Controller had given call very short time before near midair collision but used the wrong call sign for small transport so did not even get out late advisory. Small transport was long nosed twin and consequently widebody transport was in blind spot in climb to pilot of small transport. Feels controller misjudged climb rate of widebody transport and should have stopped climb. Filed near midair collision with FSS.

Narrative: WAS RECEIVING FLT FOLLOWING FROM LGA TRACON NW SECTOR AT THE TIME OF NEAR MISS. AT APPROX 10 NM NW OF LGA, ACR FLT WDB WAS CLRED TO 12000' ON 270 DEG HDG, CLBED FROM UNDER THE RIGHT FRONT NOSE OF MY ACFT THROUGH MY ALT WHEN I FIRST SAW HIM. I INITIATED AN IMMEDIATE DSCNT TO 11000' TO STAY CLR OF HIM. WE HAD BEEN ON A NEAR COLLISION COURSE. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING: LEARNED RPTR CALLED TRACON AFTER LNDG AND THEY APOLOGIZED FOR THE ERROR. CTLR WAS REMOVED FROM THE POS. RPTR LATER SAW NTAP AND TRANSCRIPT. WAS SO CLOSE SPECIALIST COULD NOT TELL WHICH SIDE OF SMT WDB PASSED AND BOTH ALT READOUTS WENT INTO COAST. CTLR HAD GIVEN CALL VERY SHORT TIME BEFORE NMAC BUT USED THE WRONG CALL SIGN FOR SMT SO DID NOT EVEN GET OUT LATE ADVISORY.
