Narrative:

Takeoff at cedar rapids, ia. ATIS reported fog and 1/16 mi visibility. Communication with the controller, they reported the RVR was 1600. Looking on the takeoff minimum box, headlined 'all runways' 1600 RVR or 1/4 mi, we proceeded to runway 27 and took off. Violation by tower for taking off below minimums was filed, since the RVR is on runway 9. Violation found out through company in dec/1988 by letter dated dec/xx/88. First officer and I have worked since dec 1988 and this is the first chance to file this report. Suggest takeoff minimum box should be broken down into individual runways, 1600 RVR for runway 9 and 1/4 mi for all other runways, to eliminate any future misunderstandings.

VIOLATION FOUND OUT THROUGH COMPANY IN DEC/1988 BY LETTER DATED DEC/XX/88. F/O AND I HAVE WORKED SINCE DEC 1988 AND THIS IS THE FIRST CHANCE TO FILE THIS REPORT. SUGGEST TKOF MINIMUM BOX SHOULD BE BROKEN DOWN INTO INDIVIDUAL RWYS, 1600 RVR FOR RWY 9 AND 1/4 MI FOR ALL OTHER RWYS, TO ELIMINATE ANY FUTURE MISUNDERSTANDINGS.
